SIUC requires all students who are taking on-campus courses to have health insurance. To meet this mandate, all students taking on-campus courses will be billed the student health insurance fee. Students who already have health insurance coverage may apply to waive the fee by completing the waiver process prior to the posted deadlines. Step-by-step instructions for completing the waiver application can be found in the FAQs below. All students taking on-campus courses who do not complete the waiver process by the posted deadline will be automatically enrolled in the SIU Student Health Insurance Plan and will not be eligible for a refund.
Attention International Students:The submission process for international students to apply for a waiver of the student health insurance fee has changed! Effective for the Fall 2024 term, international students will now be required to use the on-line waiver system managed by Gallagher Student Health. The new system is better able to identify if an outside insurance plan is compliant with ALL university health insurance requirements. This means that insurance plans approved under the previous process may now be denied if they are found to not meet ALL university health insurance requirements. It is recommended that prior to purchasing an insurance plan you confirm with the agent selling the plan that the plan meets ALL university health insurance requirements. Also, effective for the Fall term 2024, international students must upload proof of repatriation and medical evacuation coverage as part of the new waiver application process. Step-by-step instructions for submitting the waiver using the new process can be found below in the Frequently Asked Questions (FAQ) section below that is titled, “How do international students waive the insurance fee?”. |
Students who are registered for ONLY web based-distance education courses are not eligible for the Student Health Insurance Plan and are not charged the insurance fee.
If it is your intent to enroll in the student health insurance plan, it is strongly recommended that you enroll yourself as soon as possible. Doing so assures you have access to off-campus benefits on the effective date of the policy. Automatic enrollment does NOT occur until after the posted waiver deadline.
The Student Health Insurance Plan provides health insurance coverage that complements the Student Health Center, covering expenses such as pharmaceuticals, immunizations, and off-campus services such as emergency rooms, hospitalizations, surgery, ambulance, and specialty care.

- Follow the instructions to complete the form and click submit.
- You must also upload supporting documentation showing that you have medical evacuation and repatriation coverage.

- 4. How do I enroll myself? (Both Domestic & International Students)

- Under “Profile”, enter your School email address and click LOG IN.

- Click the green "Enroll" button under the "Plan Summary" tile and follow the instructions to complete the form.
- Review the information and click "Submit." Enrollment confirmation will be sent.
- 5. How do I print out my Insurance ID Card?
- Go to: www.uhcsr.com/MyAccount
- If you are a returning user, just log in with your user name and password
- If you are a new user, create a UHCSR account by clicking "Create Account"
- Complete the registration form using your name, date of birth, and SIU Dawg Tag Number
- Once logged in, you will have access to download and print your Student Health Insurance Plan ID Card, EOBs and other plan-related information.

Students: Only SIUC students taking on-campus courses are eligible for the Student Health Insurance plan. Students taking only on-line/distance education/web based courses are not eligible for the plan. SIUC faculty and staff are not eligible for the student health insurance plan. Students who are Medicare eligible prior to the beginning of the plan year are not eligible for the student health insurance plan. Students who waive the student health insurance plan are not eligible to be added back to the plan in the same year unless they experience a qualifying event such as unintentional loss of health insurance or aging out (i.e. turning 26) on another health insurance plan.
Dependents such as spouses/domestic partners and children are eligible for the student health insurance plan only through an eligible student. If a student becomes ineligible so does any associated dependents.You must be registered for at least one on-campus course for the current term before you will be able to complete the waiver process. To waive the SIU Student Health Insurance Plan, you must be enrolled in a health insurance plan with an effective date on or before the first date of class, with benefits that meet all university health insurance requirements, and complete the waiver application on or before the posted deadline. If you do not actively enroll yourself, you may not have access to your benefits until after the terms open enrollment and waiver period has ended and the automatic enrollment process is eventually completed. This active enrollment process should be completed each Fall and Spring as follows:
Students may be eligible to enroll after their open enrollment/waiver period only with a qualifying event such as losing or aging out of their insurance coverage. Contact Gallagher Student Health for the application process at 888-538-0602.
Once the enrollment process is completed you are no longer eligible to waive the plan and must remain enrolled for the entire plan year if eligible.
When you arrive on campus, take a minute to print out your Student Health Insurance Plan ID Card.

Dependent coverage must be purchased for the same time period as the students and cannot exceed the student’s period of coverage. For example, if you enroll for fall coverage, your dependents need to be enrolled for fall coverage; you wouldn’t be able to enroll them for annual coverage. If you enroll for fall coverage and do not enroll your dependents at that time, you cannot enroll your dependents unless a qualifying event occurs. Dependent enrollment must be completed by the deadline of the academic terms open enrollment/waiver period as follows:
NOTE: If enrolling a dependent for the first time in SHIP, documentation needs to be uploaded at the time of submission. For example, a marriage certificate for Dependent Spouse or Birth Certificate for Dependent Child.
Dependents may be eligible to enroll after the open enrolment/waiver period only with a qualifying event such as marriage, divorce, birth of a child, or dependent entering the country for the first time. If you waive the Student Health Insurance Plan (SHIP) and then lose coverage under that plan, you need to go to www.gallagherstudent.com/SIU. Log in, click on “Enroll-Qualifying Life Event”, complete the online form, and upload the required supporting documentation such as a birth certificate for a newborn, marriage certificate, or loss of coverage letter from your prior health insurance company showing the last day of coverage. Make sure you read the form carefully as it contains very specific information on the Qualifying Life Event process. Qualifying events include:
If your enrollment in SHIP is on a voluntary basis, there is no option for the Qualifying Life Event process if you lose coverage with your current health insurance plan.
No, you are entitled to use all the services at the Student Health Center because you were assessed the non-refundable Student Health Fee as part of fees. Most visits are charged a $10.00 door fee and can be paid by the following methods:
