Who is eligible for the plan?

Students: Only SIUC students taking on-campus courses are eligible for the Student Health Insurance plan. Students taking only on-line/distance education/web based courses are not eligible for the plan. SIUC faculty and staff are not eligible for the student health insurance plan. Students who are Medicare eligible prior to the beginning of the plan year are not eligible for the student health insurance plan. Students who waive the student health insurance plan are not eligible to be added back to the plan in the same year unless they experience a qualifying event such as unintentional loss of health insurance or aging out (i.e., turning 26) on another health insurance plan.

Dependents such as spouses/domestic partners and children are eligible for the student health insurance plan only through an eligible student. If a student becomes ineligible, so do any associated dependents.

How do I waive the fee and get a refund?

Waiver instructions

  1. Go to https://studentcenter.uhcsr.com/
  2. Type in Southern Illinois University in the search box and click the search icon.
  3. Read the important announcement and select the “WAIVE COVERAGE” option, or select “OPT-IN” if you want to enroll in the health insurance plan.
  4. Enter your birth date and your Dawg Tag number (ex. 856000000) and select “NEXT” to proceed.
  5. Review your personal information for any errors and select “NEXT” to proceed.
  6. Answer all of the waiver questions. Once all questions are answered, select “NEXT” to proceed.
  7. Complete the insurance information fields and upload proof of insurance coverage (e.g., front and back photos of insurance ID card) and select “NEXT” to proceed.
  8. Read the acknowledgement on the Sign and Submit page, and then type your full name as your signature.
  9. Click “SUBMIT”.
  10. You will receive both an on-screen and email confirmation stating that the waiver is pending verification or denied.

How do I enroll myself?

How to Opt-In to Opt-In

  1. Go to https://studentcenter.uhcsr.com/
  2. Type in Southern Illinois University in the search box and click the search icon.
  3. Read the important announcement and select Opt-In
  4. Enter your birth date and your Dawg Tag number (ex. 856000000) and select next.
  5. Review the policy term available. If there is more than one term available, only select the term you wish to enroll in.
  6. Click the Brochure link to review the policy brochure. Then click next.
  7. Make any necessary corrections to your phone number, mailing address, permanent address. Click next to proceed.
  8. Review the selected coverage and notice to students. Check the box to confirm your enrollment. The total shown is collected from you by your institution through the Student Health Insurance Fee which has already been billed to your account.
  9. Type your name on the payer signature line.
  10. Click submit.
  11. Read the acknowledgement text and type your name as your electronic signature, click submit.
  12. You will receive both an onscreen and email confirmation of your enrollment.

How do I print out my Insurance ID Card?

When you arrive on campus, take a minute to print out your Student Health Insurance Plan ID Card.

To download a Student Health Insurance Plan ID Card for United Healthcare, follow these instructions:

  • Go to: www.uhcsr.com/MyAccount
  • If you are a returning user, just log in with your user name and password
  • If you are a new user, create a UHCSR account by clicking "Create Account"
  • Complete the registration form using your name, date of birth, and SIU Dawg Tag Number
  • Once logged in, you will have access to download and print your Student Health Insurance Plan ID Card, EOBs and other plan-related information.

If I waive out of the insurance can I re-enroll?

If I waive out of the insurance, can I re-enroll?

If you waive the Student Health Insurance Plan (SHIP) and then lose coverage under that plan, you need to complete the Qualifying Life Event (pdf) form, and email it to sidhelp@uhcsr.com along with the required supporting documentation such as a birth certificate for a newborn, marriage certificate, or loss of coverage letter from your prior health insurance company showing the last day of coverage. Make sure you read the form carefully as it contains very specific information on the Qualifying Life Event process. Qualifying events include:

  • Reaching the age limit of another health insurance plan
  • Loss of health insurance through marriage or divorce
  • Involuntary loss of coverage from another health insurance plan

If your enrollment in SHIP is on a voluntary basis, there is no option for the Qualifying Life Event process if you lose coverage with your current health insurance plan.

Do I need Health Insurance to access the Student Health Center?

No, you are entitled to use all the services at the Student Health Center because you were assessed the non-refundable Student Health Fee as part of fees. Most visits are charged a $10.00 door fee and can be paid by the following methods:

Is the Student Health Insurance Plan primary or secondary coverage?

The Student Health Insurance Plan is a coordinated benefits plan and, in most situations, will be primary.

Do I have to apply for an insurance waiver every semester?

Once a refund is requested online in the Fall semester and granted, you are exempt for one Academic year. If you are entering SIU in either the Spring or Summer semesters, you may apply for the refund for that semester but will need to apply again in the Fall. Then you will be exempt for one year.
