- 1. Who is eligible for the plan?
Students: Only SIUC students taking on-campus courses are eligible for the Student Health Insurance plan. Students taking only on-line/distance education/web based courses are not eligible for the plan. SIUC faculty and staff are not eligible for the student health insurance plan. Students who are Medicare eligible prior to the beginning of the plan year are not eligible for the student health insurance plan. Students who waive the student health insurance plan are not eligible to be added back to the plan in the same year unless they experience a qualifying event such as unintentional loss of health insurance or aging out (i.e. turning 26) on another health insurance plan.
Dependents such as spouses/domestic partners and children are eligible for the student health insurance plan only through an eligible student. If a student becomes ineligible so does any associated dependents.
- 2. How do I waive the fee and get a refund? (For Students who are permanent US Residents)
You must be registered for at least one on-campus course for the current term before you will be able to complete the waiver process. To waive the SIU Student Health Insurance Plan, you must be enrolled in a health insurance plan with an effective date on or before the first date of class, with benefits that meet all university health insurance requirements, and complete the waiver application on or before the posted deadline. Instructions for completing the waiver process are as follows:
- Click "ENROLLMENT/WAIVER LINK"
- Under “Profile,” enter your School email address and click LOG IN
First-time users: You should have received an email from Gallagher Student Health with a temporary password. Click on the link provided in the email and insert the temporary password. (If you did not receive a temporary password, you can contact Gallagher Student Health at 1-888-538-0602.).
- Select the yellow "Waive" button under the "Plan Summary" tile
- You will need your health insurance information to answer the questions.
- Follow the instructions to complete the form and click submit.
- A reference number will be emailed upon submission. However, a final determination may take 24-48 hours.
Waiving the student health insurance plan does not exclude access to the on-campus Student Health Center.

- 3. How do International Students waive the Insurance Fee?

- Select the yellow "Waive" button under the "Plan Summary" tile
- You will need your health insurance information to answer the questions.
- Follow the instructions to complete the form and click submit.
- You must also upload supporting documentation showing that you have medical evacuation and repatriation coverage.
- A reference number will be emailed upon submission. However, a final determination may take 24-48 hours.
Waiving the student health insurance plan does not exclude access to the on-campus Student Health Center.

- 4. How do I enroll myself? (Both Domestic & International Students)
If it is your intent to enroll in the student health insurance plan, it is strongly recommended you complete the online enrollment form as soon as possible. Doing so assures you have access to benefits on the effective date of the policy. If you do not actively enroll yourself, you may not have access to your benefits until after the terms open enrollment and waiver period has ended and the automatic enrollment process is eventually completed. This active enrollment process should be completed each Fall and Spring as follows:

- Under “Profile”, enter your School email address and click LOG IN.
First-time users: You should have received an email from Gallagher Student Health with a temporary password. Click on the link provided in the email and insert the temporary password. (If you did not receive a temporary password, you can contact Gallagher Student Health at 1-888-538-0602.)
- Click the green "Enroll" button under the "Plan Summary" tile and follow the instructions to complete the form.
- Review the information and click "Submit." Enrollment confirmation will be sent.
Students may be eligible to enroll after their open enrollment/waiver period only with a qualifying event such as losing or aging out of their insurance coverage. Contact Gallagher Student Health for the application process at 888-538-0602.
Once the enrollment process is completed you are no longer eligible to waive the plan and must remain enrolled for the entire plan year if eligible.
- 5. How do I print out my Insurance ID Card?
When you arrive on campus, take a minute to print out your Student Health Insurance Plan ID Card.
To download a Student Health Insurance Plan ID Card for United Healthcare, follow these instructions:
- Go to: www.uhcsr.com/MyAccount
- If you are a returning user, just log in with your user name and password
- If you are a new user, create a UHCSR account by clicking "Create Account"
- Complete the registration form using your name, date of birth, and SIU Dawg Tag Number
- Once logged in, you will have access to download and print your Student Health Insurance Plan ID Card, EOBs and other plan-related information.
- 6. How do I enroll dependents?
Dependent coverage must be purchased for the same time period as the students and cannot exceed the student’s period of coverage. For example, if you enroll for fall coverage, your dependents need to be enrolled for fall coverage; you wouldn’t be able to enroll them for annual coverage. If you enroll for fall coverage and do not enroll your dependents at that time, you cannot enroll your dependents unless a qualifying event occurs. Dependent enrollment must be completed by the deadline of the academic terms open enrollment/waiver period as follows:

- Click on the ‘Enroll’ button under ‘Plan Summary’.
- Follow the instructions to complete the form and submit payment.
NOTE: If enrolling a dependent for the first time in SHIP, documentation needs to be uploaded at the time of submission. For example, a marriage certificate for Dependent Spouse or Birth Certificate for Dependent Child.
Dependents may be eligible to enroll after the open enrolment/waiver period only with a qualifying event such as marriage, divorce, birth of a child, or dependent entering the country for the first time. Contact Gallagher Student Health for application process at 888-538-0602.
- 7. If I waive out of the insurance can I re-enroll?
If I waive out of the insurance, can I re-enroll?
If you waive the Student Health Insurance Plan (SHIP) and then lose coverage under that plan, you need to go to www.gallagherstudent.com/SIU. Log in, click on “Enroll-Qualifying Life Event”, complete the online form, and upload the required supporting documentation such as a birth certificate for a newborn, marriage certificate, or loss of coverage letter from your prior health insurance company showing the last day of coverage. Make sure you read the form carefully as it contains very specific information on the Qualifying Life Event process. Qualifying events include:
- Reaching the age limit of another health insurance plan
- Loss of health insurance through marriage or divorce
- Involuntary loss of coverage from another health insurance plan
If your enrollment in SHIP is on a voluntary basis, there is no option for the Qualifying Life Event process if you lose coverage with your current health insurance plan.
- 8. Do I need Health Insurance to access the Student Health Center?
No, you are entitled to use all the services at the Student Health Center because you were assessed the non-refundable Student Health Fee as part of fees. Most visits are charged a $10.00 door fee and can be paid by the following methods:
- cash
- check
- credit or debit card
- or bill to your Bursar
Charges vary for Pharmacy services.
- 9. Is the Student Health Insurance Plan primary or secondary coverage?
The Student Health Insurance Plan is a coordination of benefits plan and in most situations will be primary.
- 10. Do I have to apply for an insurance waiver every semester?
Once a refund is requested online in the Fall semester and granted, you are exempt for one Academic year. If you are entering SIU in either the Spring or Summer semesters, you may apply for the refund for that semester but will need to apply again in the Fall. Then you will be exempt for one year.
